1. HIGHLIGHTS

Thirty-three (33) new confirmed cases including 6 community deaths were reported during the day of 04 July 2026, in the provinces of Ituri (30 cases including 6 deaths) and for Nord-Kivu (3 cases). These confirmed cases come from the ZS (health zone) of Rwampara (11), Bunia (7), Nizi (7), Komanda (3), Mongbwalu (2) in Ituri and from Musienene (2) and Katwa (1) in Nord-Kivu.

Eight (8) deaths were recorded among MVE (Ebola virus disease) patients in the CTE (Ebola treatment centre) in the provinces of Ituri (2 at CTE Bunia, 2 at CTE Mongbwalu, 2 at CTE Elikya and 1 at CTE Komanda) and Nord-Kivu (1 at CTE Katwa).

Fifteen (15) patients suffering from Ebola virus disease in the province of Ituri (4 at CTE Rwampara, 3 at CTE Mongbwalu, 3 at CTE Bunia, 2 at CTE Komanda, 1 at CTE CME, 1 at CTE Elikya and 1 at CTE Nyankunde) were declared cured after obtaining negative control tests.

Beginning of clinical trials at CTE CME in the Rwampara health zone in Ituri.

2. EPIDEMIOLOGICAL AND OPERATIONAL CONTEXT

Ituri province, located in the north-east of the Democratic Republic of the Congo, shares a long and porous border with Uganda and South Sudan. For more than two decades, it has been facing a chronic humanitarian crisis linked to armed conflicts and recurrent population displacements. Its population is estimated at more than 8 million inhabitants, including more than one million internally displaced persons. The Mongbwalu health zone, considered the starting point of the outbreak, is located in Djugu territory, 70 km from Bunia. This zone is characterised by the presence of armed groups, frequent population movements towards Uganda, and numerous artisanal mining sites attracting migrant workers. In September 2018, Ituri province had already been affected by the 10th MVE outbreak, which was then raging in Nord-Kivu. The current outbreak, caused by Bundibugyo ebolavirus, was officially declared on 15 May 2026 by the Minister of Public Health. The three affected provinces have a total population estimated at nearly 15 million inhabitants, with massive internal displacements and cross-border flows towards neighbouring countries.

Total 1 561 506 32,4% 36 out of 104 (34,6 %) 33

*7 deaths in Ituri and 1 death in Nord-Kivu were recorded among MVE patients (previous confirmed cases) in the CTE.

At the end of the day of 04 July 2026, no new health zone was affected. The number of affected health zones remains at 36/104 (section 3.1). Since 26 May 2026, Sud-Kivu province has recorded no confirmed case, suggesting an absence of recent transmission.

3.3 Temporal analysis:

An increasing number of confirmed cases is observed from one week to the next, reflecting continuous transmission of the disease in the community, although during epidemiological weeks 23 and 24, the number of cases appeared to be stable. The intensification of public health actions in terms of epidemiological and biological surveillance (decentralisation of diagnostic capacities) contributed to early detection, confirming continuous and increased community transmission in W25. Compared to all weeks where cases have been recorded since the start of the outbreak, W25 and W26 represent the peak so far, with more than 300 weekly cases.

Over the last 24h, 33 new confirmed cases and 14 deaths were recorded in the provinces of Ituri (30 cases and 13 deaths) and Nord-Kivu (3 cases and 1 death), highlighting the progression of the epidemiological situation of the 17th Ebola Virus Disease (MVE) outbreak in the Democratic Republic of the Congo. Thus, the cumulative total stands at 1 561 confirmed cases and 506 deaths, reflecting an overall case fatality rate of 32,4%.

Provincial dynamics and concentration of cases

Ituri province alone accounts for 90,8% of cases (n=1 417) and 83,5% of deaths (n=424), representing a case fatality rate of 29,9%, making it the absolute epicentre of the outbreak. The health zones of Bunia (438 cases), Rwampara (344 cases) and Mongbwalu (278 cases) remain the most affected and account for approximately 74,8% of cases reported at the provincial level and 67,9% at the national level. In these same ZS, moderate case fatality rates were observed in the ZS of Rwampara (20,6%) and Bunia (26,9%), while the Mongbwalu zone also combines a high case fatality rate, namely 50,7% (141 deaths for 278 cases), suggesting persistent challenges in early case management or access to care. It should be noted that 17 additional cases (with no associated death) were reported while the health zones of origin remain to be identified, which could indicate a geographical spread requiring reinforced investigation.

Severity of the outbreak in Nord-Kivu

A total of 141 confirmed cases and 81 deaths has been reported in Nord-Kivu province since the beginning of the outbreak, representing a case fatality rate of 57,4%. The latter presents the most concerning profile in terms of severity, with a case fatality rate well above the national average. This high case fatality rate is driven by the health zones of Katwa (73,1%), Oicha (66,7%), Beni (57,1%), Kalunguta, Kyondo and Musienene (50,0%) respectively. The Katwa health zone leads notifications in terms of case numbers (n=52), followed by the Butembo ZS (n=40) with a case fatality rate of 45,0%. A case fatality rate of 100% was exceptionally observed in the Vuhovi health zone (with small numbers).

5. MAIN CHALLENGES, IMPACTS AND REQUIRED ACTIONS

No. Challenges Impact Required actions
1 Reluctance towards post-mortem sampling (EDS / swabs) and community resistance Delayed confirmation, underestimation of cases, disruption of key activities Intensify CREC, involvement of religious and community leaders, strengthening of team safety
2 Insufficient CTE capacity and saturation in Nord-Kivu (138,8%) Admission delays, increased risk of nosocomial transmission Accelerate construction and expansion of CTEs, deploy additional temporary structures
3 Non-optimal contact tracing performance (81,3% vs 95% target) Transmission chains not interrupted Train, deploy and motivate community health workers (RECO), reinforced supervision
4 Low reporting of alerts in certain affected ZS Undetected transmission, silent expansion of the outbreak Strengthen community surveillance, improve prompt reporting and investigation of alerts
5 Confirmed cases not yet broken down by health zone (17 cases) Major gaps in the reconstruction of transmission chains Conduct data reconciliation, prune possible duplicates, and reclassify or clarify the health zone and status (deaths, cured, active, etc.) Active verification in the field
6 Insufficient supply of essential generic drugs and medical inputs Weakening of overall care Advocacy and urgent supply of essential medicines
7 Insufficient PCI inputs (PPE, chlorine) and limited training High risk of nosocomial infections (96 HCW cases including 19 deaths, 19,7% case fatality) Urgent supply, strengthening of PCI training
8 Stock of samples not yet analysed (backlog) at the laboratory in Nord-Kivu and late diagnosis Delayed confirmation, high case fatality (54,2%) Deploy a laboratory catch-up plan, strengthen diagnostic capacities
9 Insufficient ambulances and isolation structures (gap of 20 centres) Delays in transfer and isolation, increased risk of transmission Urgent deployment of logistics resources and additional infrastructure
10 Insufficient coordination and low use of COUSP frameworks Operational inefficiency and delayed decisions Strengthen coordination mechanisms at all levels, operationalise provincial COUSPs
