*Data harmonisation with the provinces of Ituri and Nord-Kivu as of 22 July 2026.

The significant increases observed in the cumulative totals result mainly from the integration of the harmonised provincial databases and do not solely reflect new notifications from the last 24 hours.

1 Epidemiological highlights

• Epidemiological situation: No new affected health zone has been reported. The cumulative total remains at 47 affected health zones out of 140 in the five provinces, of which 46 remain in active transmission.

• Coordination: Arrival of the new National Incident Manager (IM) in Bunia, Professor Steve Ahuka; The Minister of Public Health, Hygiene and Social Welfare as well as Professor Pierre Akilimali (IM) were received in audience by the President of Uganda on 22 July 2026 to review the situation of the Bundibugyo Ebola Virus Disease (MVE-B 17); Harmonisation of epidemiological data between the provinces of Ituri and Nord-Kivu and the national level.

• Evolution of cases: During the last 24 hours, 97 new confirmed cases, 62 confirmed deaths and 13 recoveries were recorded:

o Ituri: 73 new confirmed cases were reported, mainly in Nizi (24), Rwampara (14), Bania (10), Nia-Nia (8), Bambu (4), Fataki (4), Logo (2), Mangala (2) Mongbwalu (2), Lita (2) and Komanda (1), 45 deaths among which, 73,3% community.

The provincial cumulative total reaches 275 confirmed cases and 172 deaths, representing a case fatality rate of 62,5%.

o Haut-Uélé: 6 new confirmed cases and 3 deaths of which 66,7% community were reported. The provincial cumulative total rises to 27 confirmed cases and 15 deaths, representing a case fatality rate of 55,6 %.

o Sud-Kivu and Tshopo: No new confirmed case nor death was reported. The cumulative totals remain at 5 cases, 4 deaths and 1 recovered in Tshopo and 3 cases of which 1 death in Sud-Kivu, where the Miti-Murhesa health zone totals 55 days without a new confirmed case.

• Analysis: Ituri remains the epicentre of the outbreak, concentrating 89,3 % of cumulative cases and 75,3 % of new confirmations of the day. Contact tracing performance remains insufficient and requires a strengthening of surveillance activities.

EPIDEMIOLOGICAL AND OPERATIONAL CONTEXT

Declared on 15 May 2026, the 17th outbreak of Ebola virus disease (MVE) due to Bundibugyo ebolavirus continues its course in a complex humanitarian and security context. Initially confined to the health zone of Mongbwalu (Ituri), it spread to five provinces (Ituri, Nord-Kivu, Sud-Kivu, Haut-Uélé and Tshopo), now affecting 47 health zones. Population movements, insecurity, artisanal mining activities and cross-border trade with Uganda and South Sudan continue to favour transmission. Multisectoral coordination remains mobilised in order to strengthen surveillance, intensify response interventions and limit the spread of the outbreak.

2.3 Mapping of confirmed cases

The spatial distribution confirms transmission mainly concentrated in Ituri, particularly in the health zones of Bunia, Rwampara, Mongbwalu, Nizi, Nyankunde and Lita, with continued transmission in the already affected zones of Haut-Uélé.

[FIGURE: FIGURE 2 — HEALTH ZONES THAT REPORTED CONFIRMED CASES (AS OF 22/07/2026)]

The recent appearance of new foci in Haut-Uélé confirms a progressive extension of the affected zones, highlighting the need to maintain simultaneously intensive interventions at the level of the epicentre and preparedness activities in neighbouring provinces.

2.4 Temporal analysis — epidemic curves

The epidemic curve shows sustained transmission during weeks W25 to W30, with fluctuations linked to reporting delays and the progressive consolidation of data.

[FIGURE: FIGURE 3 — EPIDEMIC CURVE BY WEEK OF NOTIFICATION]

[FIGURE: FIGURE 4 EPIDEMIC CURVE BY DATE OF ONSET OF SYMPTOMS (DHIS2 LINE LIST)]

The outbreak remains in a phase of sustained transmission, with a high level of virus circulation. Recent fluctuations must be interpreted with caution due to reporting delays and the progressive consolidation of data.

2.5 Demographic characteristics

The age pyramid shows a predominance of working-age adults (18–50 years). Minors under 18 years and people aged over 50 years are proportionally less affected.

[FIGURE: FIGURE 5 — CONFIRMED CASES BY SEX AND AGE GROUP]

[FIGURE: FIGURE 6 — DEATHS BY SEX AND AGE GROUP (AMONG CONFIRMED CASES)]

The distribution of cases by sex remains balanced. The predominance of cases among working-age adults reflects more frequent community and occupational exposures, whilst the proportion of paediatric cases testifies to the persistence of intrafamilial transmission.

3 Surveillance, testing and transmission

3.1 Alert management and sources

In total, 1 484 alerts (of which 1 323 new), 1 179 investigated within 24h (79,5%) and 318 validated (203 live and 115 community deaths). This improvement reflects the reduction in investigation carryovers and the consolidation of data from the affected provinces. The improvement in the investigation rate is linked to the reduction in investigation carryovers and the strengthening of surveillance teams in the affected provinces.

3.2 Laboratory: testing and positivity

8 Main challenges, impacts and required actions

N° Challenges Impact Required actions
1 Reluctance towards post-mortem sampling (EDS/swabs), community resistance and insecurity targeting EDS teams (attack at Muchanga bridge, paralysis of activities in Nyankunde) Delay in confirmation, underestimation of cases, disruption of key activities Intensify RCCE, involve religious and community leaders, secure teams, increase the number of EDS teams
2 Non-payment of providers (strike in the ZS of Bunia and Rwampara; resumption announced following the intervention of the military governor of Ituri) Interruption of response activities, including safe and dignified burials Regularise payment of providers; secure dedicated funding
3 Insufficient CTE/CT capacity: occupancy of 119,1 % in Nord-Kivu (bed capacity: 141) and 77,7 % in Ituri (551 patients for 709 beds); Admission delays, increased risk of nosocomial transmission Accelerate construction and expansion of CTEs, deploy temporary structures, systematically report bed capacity
4 Contact tracing performance low in Ituri 73,3 % ; Nord-Kivu 77,7% Uninterrupted transmission chains Train, deploy and motivate community health workers (RECO); strengthened supervision; investigate the decline observed in Ituri
5 Shortage of ambulances and prepositioning at PoCs Delays in transfer and isolation Preposition ambulances (including at PoE/PoC), deploy CTs by ZS
6 Insufficient supply of essential generic drugs (MEG) and medical supplies Weakening of overall care Advocacy and urgent supply of essential medicines
7 Shortage of IPC supplies (PPE, chlorine) and limited functional triage units (175/1 170, or 15 %) High risk of nosocomial infections (121 PPL cases including 36 deaths) Urgent supply, strengthening IPC training
8 Geographical extension (Haut-Uélé, Tshopo) and high mobility Risk of urban, river and cross-border spread Activate the 23 priority PoE/PoC in Haut-Uélé; strengthen cross-border coordination
9 Installation of local laboratories (Butembo, Mambasa and Logo) Long confirmation times, undetected transmission Accelerate the deployment of decentralised diagnostic capacities
10 Insufficient financial resources Limitation of pillar implementation Urgent resource mobilisation, advocacy with partners
11 Insecurity and limited access (CODECO, ADF) Restriction of operations Strengthening security coordination and humanitarian access
12 Continuity of essential health services (CEHS) compromised Increase in indirect mortality (non-Ebola) Effective implementation of free healthcare, strengthening CEHS
