1 Epidemiological highlights

• Epidemiological situation: No new health zone was affected in the course of the last 24 hours. The outbreak remains circumscribed to 51 health zones out of 140, distributed across five provinces: Ituri (28/36), Nord-Kivu (12/34), Haut-Uélé (5/13), Tshopo (5/23) and Sud-Kivu (1/34). Nord-Kivu has 73 affected health areas out of 684 and Haut-Uélé 22 out of 213.

• Case evolution: In the course of the last 24 hours, 72 new confirmed cases, 44 confirmed deaths and 22 recoveries were recorded, bringing the cumulative totals to 3 874 confirmed cases, 1 751 deaths and 749 recovered, representing a case fatality rate of 45,1 %. The deaths of the day are divided into 30 community deaths confirmed by post-mortem sampling (66,7 %) and 14 deaths occurring in CTE (Ebola treatment centre).

o Ituri: 58 new confirmed cases were returned by the laboratory, notified in Rwampara (19), Bunia (11), Nia-Nia (8), Nizi (8), Bambu (5), Tchomia (2), Komanda (1), Lita (1), Logo (1), Mangala (1) and Mongbwalu (1), as well as 35 deaths including 25 community deaths (71,4 %). Six positive repeat samples were also returned, bringing the day’s cumulative total of positives to 64.

o Nord-Kivu: 12 new confirmed cases in Katwa (9), Butembo (2) and Musienene (1), and 5 deaths, all recorded in Katwa, including 2 occurring in CTE. The cumulative total reaches 426 cases and 288 deaths, representing a case fatality rate of 68,1 %.

o Haut-Uélé: 2 new confirmed cases in Isiro, both deceased and confirmed by Swab, as well as 2 in-hospital deaths reported in Boma Mangbetu and in Pawa. The province totals 63 confirmed cases and 38 deaths, representing a case fatality rate of 60,3 %, distributed across 5 health zones out of 13 and 22 health areas out of 213. The escaped confirmed case from Gbonzunzu was found in the AS (health area) of Adjangwe.

o Tshopo: No new confirmed cases nor deaths were reported. The provincial cumulative total remains at 7 confirmed cases, 5 deaths and 1 recovered, representing a case fatality rate of 71,4 %.

o Sud-Kivu: The provincial cumulative total remains at 3 confirmed cases including 1 death and 2 recovered. The province totals 54 days without a new confirmed case since 10 juin 2026. Contact tracing is maintained: 78 contacts listed, 23 discharged and 55 undergoing monitoring, all seen (100 %),

• Analysis: Ituri remains the epicentre with 87,1 % of cumulative cases and 80,6 % of the day’s confirmations, but its case fatality rate (42,0 %) remains to that of Nord-Kivu (67,6 %), which accounts for 16,7 % of new cases. This gap reflects very late detection in Nord-Kivu, where 51 of the 109 suspected cases of the day are deaths (46,8 %) and where intra-CTE mortality reaches 35 %. Nationally, 28 of the 44 deaths of the day (63,6 %) occurred in the community: transmission still largely escapes the early detection mechanism. Contact tracing is at 78,4 % compared to 79,2 % the previous day, below the operational threshold of 95 %; this decrease is very marked in the province of Ituri (80,2 %), where Bunia (32,2 %) and Mangala (53,0 %) alone account for 1 771 of the 2 289 unseen contacts.

EPIDEMIOLOGICAL AND OPERATIONAL CONTEXT

Declared on 15 mai 2026, the 17th outbreak of Ebola virus disease (MVE) due to Bundibugyo ebolavirus continues its progression in a complex humanitarian and security context. Initially circumscribed to the health zone of Mongbwalu (Ituri), it spread to five provinces (Ituri, Nord-Kivu, Sud-Kivu, Haut-Uélé and Tshopo), now affecting 51 health zones. Population movements, insecurity, artisanal mining activities and cross-border trade with Uganda and South Sudan continue to promote transmission. Multisectoral coordination remains mobilised in order to strengthen surveillance, intensify response interventions and limit the spread of the epidemic.

2.3 Mapping of confirmed cases

The spatial distribution confirms transmission mainly concentrated in Ituri, notably in the health zones of Bunia, Rwampara, Mongbwalu, Nizi, Lita and Nyankunde, transmission continuing in already affected zones without new geographical extension in the course of the last 24 hours.

[FIGURE: FIGURE 2 — SPATIAL DISTRIBUTION OF HEALTH ZONES HAVING REPORTED CONFIRMED CASES]

Transmission in already affected health zones, underlining the need to simultaneously maintain intensive interventions at the epicentre and preparedness activities in neighbouring provinces.

2.4 Temporal analysis — epidemic curves

The epidemic curve shows sustained transmission during weeks W26 to W31, with fluctuations linked to reporting delays and the progressive consolidation of data.

[FIGURE: FIGURE 3 — EPIDEMIC CURVE BY WEEK OF NOTIFICATION]

[FIGURE: FIGURE 4 EPIDEMIC CURVE BY DATE OF SYMPTOM ONSET (DHIS2 LINE LIST)]

The outbreak remains in a phase of sustained transmission, with a high level of virus circulation. Recent fluctuations must be interpreted with caution due to reporting delays and the progressive consolidation of data.

2.5 Demographic characteristics

The age pyramid shows a predominance of working-age adults (18–50 years). Minors under 18 years and people aged over 50 years are proportionally less affected.

[FIGURE: FIGURE 5 — CONFIRMED CASES BY SEX AND AGE GROUP]

[FIGURE: FIGURE 6 — DEATHS BY SEX AND AGE GROUP (AMONG CONFIRMED CASES)]

The distribution of cases by sex remains balanced. The predominance of cases among working-age adults reflects more frequent community and occupational exposures, whilst the proportion of paediatric cases bears witness to the persistence of intrafamilial transmission.

→ Ituri: 21 newly recovered; 471 patients in isolation (302 confirmed, 169 suspects) out of 833 beds, representing 56,5 % occupancy, and 57 severe or critical forms, for 77 admissions and 75 discharges. Two CTEs exceed saturation for confirmed cases — Nizi (272 %) and ISTM

feedbacks captured, 37 were clarified (36 %). One refusal persists in Rwampara and one reluctance in Nizi, where a 60-year-old woman having lost 10 members of her family refuses her transfer; the resistance in Nyankunde, linked to the delay of EDS teams, has not been lifted.

→ Nord-Kivu: 19 379 households visited out of 25 500 (75,9 %) and 32 605 people reached door-to-door, 3 199 during dialogues and talks and 3 803 in public places; 62 radio stations out of 72 broadcast (86,1 %) and 165 alerts of suspected cases were raised out of 450 expected (36,6 %); 6 feedbacks out of 8 resolved within 48 hours and the 2 recorded incidents lifted. Mass communication in Butsili (Beni, 81 participants), community dialogue in Mbimbi (Oicha, 42 breastfeeding women) and interactive broadcast on the radio of Kantine (Biena).

→ Tshopo: awareness-raising at the central market of Kisangani by youths from URNSC (1 100 participants), support to the training of IT of the five ZS of the city (92 participants) and stakeholders from the river police and boat owners (85 participants); 17 community alerts collected and continuation of communication around the cases of Kongakonga and Kabondo.

→ Sud-Kivu: the door-to-door community-based communication strategy totals 44 585 households visited out of 57 638 (77 %), 121 patients referred to health centres and 4 visitors arriving from affected zones identified; Nyangezi reaches 100 % coverage across its six health areas, Ibanda 68 %.

→ Haut-Uélé: 1 271 people reached at home in 458 households and 5 500 in mass awareness-raising, including support from the governor in Vube (Rungu) and in chiefdom Mayogo-Magbai; 4 radio stations out of 14 broadcast and 50 IEC tools were disseminated, bringing the cumulative total to 285. Educational talk with the 11 family members of the deceased at Clinique de l’Est, awareness-raising of 38 members of the ECZ of Pawa and talks around 2 recovered persons in Wamba; 7 feedbacks recorded, of which 5 captured.

(140 %) — and one for suspected cases (Aru 100 %). Nutritional support: 3 patients on specific diet and 1 under management for severe acute malnutrition.

→ Nord-Kivu: 4 admissions of confirmed cases and 18 of suspected cases for 2 deaths and 9 non-case discharges, none recovered; 196 patients in isolation at the end of the day (27 confirmed, 169 suspects) for 141 beds. Evaluation mission at the CTE of Katwa, Matanda hospital and CUG. On the nutrition component, recruitment of 27 nutritionists for 9 ZS (IEDA Relief / UNICEF) and joint supervision APATEMACO/WFP in Butembo, Katwa, Beni and Oicha; Masereka and Mabalako remain without nutritional care.

→ Tshopo: one new admission — a 13-year-old female suspect at the CT of HC Kisangani — for 3 patients in isolation at the end of the day (1 confirmed, 2 suspects) out of 20 beds (15,0 %); continuation of experimental treatments in 2 high-risk contacts of PPL in Madula, one of whom discharged, and in occupational contacts of HC Kisangani, of which one treatment interrupted. Eight high-risk contacts of Kongakonga refuse treatment. Nine hot meals served at the CTE.

→ Haut-Uélé: 31 patients in isolation at the end of the day for 68 beds (45,6 %); the day’s deck presents no patient management table, admission and discharge movements are therefore not documented. Four suspected cases were transferred to the CT/CTE of Isiro and three suspected cases from AS Babonde evacuated to the HGR of Pawa, where installation of a transit centre is deemed urgent.

RESEARCH, INNOVATION & VACCINATION

→ Ituri: deployment of investigators to the field with the support of Africa CDC and preparation of qualitative research in the ZS of Bunia and Rwampara; 11 protocols validated by the ethics committee (100 %), 3 studies completed out of 11 planned (27,2 %), 3 reports produced and 1 publication; 4 recommendations implemented out of 22 (22,7 %). No vaccination data is reported.

SAFE AND DIGNIFIED BURIALS (EDS)

→ Ituri: 78 death alerts, 69 EDS completed including 13 in CTE, and 55 bodies swabbed; 13 zones out of 32 reported (40,6 %). Resistance persists in Bambu (refusal of the family) and Mambasa (disbelief in MVE). The cumulative total reaches 2 779 EDS for 3 424 alerts since 20 mai. Start of training of two additional teams in Lita.

→ Nord-Kivu: 41 death alerts, 41 bodies swabbed and secured, 42 EDS completed and no failures; 15 bodies remain pending. Deaths occurring in ESS dominate, in Beni (14) and Katwa (12). The PCI pillar reports for its part 80 swabs and 52 EDS on the same day — discrepancy to be reconciled.

→ Haut-Uélé: 5 death alerts received (Wamba 2, Isiro 2, Pawa 1), 5 bodies swabbed and 5 EDS completed, no failures or postponements; acceptance of the two bereaved families of Isiro. Stockout of chlorine and delay in digging graves.

→ Sud-Kivu: no community death nor post-mortem swab reported for the day; 16 suspected cases are cared for at the CT (Katana 7, Kadutu 5, Miti-Murhesa 4) and 44 contacts remain in cantonment.

PREVENTION OF SEXUAL EXPLOITATION AND ABUSE (PSEA / PRSEAH)

→ Ituri: start of PSEAH/GBV briefings and community awareness-raising in the ZS of Bambu thanks to trained focal points; briefing of actors and signature of code of conduct by 20 participants (2 women, 18 men) from the ECZ and pillars of the ZS of Fataki. Challenges: incomplete coverage of affected zones and absence of complaints mechanisms (CBCM/CFM) in several of them.

7 Main challenges, impacts and required actions

N° Challenges Impact Required actions
1 Reluctance towards post-mortem sampling (EDS/swabs), community resistance and insecurity targeting EDS teams (attack at Muchanga bridge, paralysis of activities in Nyankunde) Confirmation delay, underestimation of cases, disruption of key activities Intensify CREC, involve religious and community leaders, secure teams, increase the number of EDS teams
2 Non-payment of providers (strike in the ZS of Bunia and Rwampara; resumption announced following the intervention of the military governor of Ituri) Interruption of response activities, including safe and dignified burials Regularise payment of providers; secure dedicated funding
3 Insufficient contact tracing performance: Ituri 83,4 %, Nord-Kivu 75,0 % and Sud-Kivu 100 %, Haut-Uélé and Tshopo having not reported; the ZS of Damas, Logo and Kambala (Ituri) remained without usable data Unbroken transmission chains Train, deploy and motivate community health workers (RECO); reinforced supervision; consolidate the recovery observed in Ituri
4 Insufficiency of ambulances and prepositioning at PoCs Delays in transfer and isolation Preposition ambulances (including at PoE/PoC), deploy CTs per ZS
5 Insufficiency of PCI supplies (PPE, chlorine) and limited functional triage units (181/1170, or 15 %) High risk of nosocomial infections (140 PPL cases including 41 deaths in Ituri; 11 PPL including 3 deaths in Nord-Kivu and 3 confirmed PPL in Haut-Uélé) Urgent supply, strengthening of PCI training
6 Geographical extension (Haut-Uélé, Tshopo) and high mobility Risk of urban, riverine and cross-border spread Activate the 23 priority PoE/PoCs of Haut-Uélé; strengthen cross-border coordination
7 Installation of proximate laboratories (Rwampara, Fataki, Bambu-mine, Boga and Mambasa) Long confirmation turnaround times, undetected transmission Accelerate deployment of decentralised diagnostic capacities
8 Insufficiency of financial resources Limitation of pillar implementation Urgent resource mobilisation, advocacy with partners
9 Insecurity and limited access (CODECO, ADF) Operations restriction Strengthening of security coordination and humanitarian access
10 Continuity of essential services (CEHS) compromised Increase in indirect mortality (non-Ebola) Effective implementation of free healthcare, strengthening CEHS
