1 Epidemiological highlights

• Epidemiological situation: No new health zone was reached over the past 24 hours.

The outbreak remains confined to 51 health zones out of 140, distributed across five provinces: Ituri (28/36), Nord-Kivu (12/34), Haut-Uélé (5/13), Tshopo (5/23) and Sud-Kivu (1/34).

Nord-Kivu has 74 affected health areas out of 684 and Haut-Uélé 22 out of 213.

• Evolution of cases: Over the past 24 hours, 99 new confirmed cases, 52 confirmed deaths and 27 recoveries were recorded, bringing the cumulative totals to 3 973 confirmed cases, 1 801 deaths and 776 recovered, representing a case fatality ratio of 45,3 %.

The deaths of the day are broken down into 37 community deaths confirmed by post-mortem sampling (71,2 %) and 15 deaths occurring in a CTE (Ebola treatment centre).

The national cumulative death toll increases by only 50 units however: the provincial total for Nord-Kivu published in the previous edition exceeded the sum of its health zones by two units, a discrepancy resolved in the present edition.

o Ituri: 85 new confirmed cases were returned by the laboratory, reported in Bunia (25), Nizi (21), Rwampara (18), Lita (9), Mongbwalu (5), Fataki (5) and Mangala (2), as well as 47 deaths of which 35 were community deaths (74,5 %).

The 12 deaths occurring in a CTE are distributed among Rwampara (3), Nia-Nia (3), Bambu (2), Bunia (1), Mongbwalu (1), Nizi (1) and Nyankunde (1).

Six (6) positive repeat samples were also returned, bringing the total positives of the day to 91.

o Nord-Kivu: 10 new confirmed cases in Katwa (5), Butembo (3), Kalunguta (1) and Musienene (1), and 4 deaths, all recorded in Katwa.

o Haut-Uélé: 4 new confirmed cases in Isiro, of which 3 are living and 1 death confirmed by swab.

o Tshopo: No new confirmed cases or deaths were reported.

The provincial cumulative total remains at 7 confirmed cases, 5 deaths and 1 recovered, representing a case fatality ratio of 71,4 %.

o Sud-Kivu: The provincial cumulative total remains at 3 confirmed cases of which 1 death and 2 recovered, to which are added 2 probable cases.

The province totals 56 days without a new confirmed case since 10 June 2026.

• Analysis: Ituri remains the epicentre with 87,1 % of cumulative cases and 85,9 % of the day’s confirmations, and its case fatality ratio (42,3 %) remains well below that of Nord-Kivu (67,0 %), which now accounts for only 10,1 % of new cases.

This gap probably reflects later detection in Nord-Kivu, where intra-CTE mortality reaches 36 %.

Nationally, 37 of the 52 deaths of the day (71,2 %) occurred in the community, compared with 63,6 % the day before.

Contact tracing falls back to 75,3 % compared with 78,4 % the day before, far from the operational threshold of 95 %; the decrease is driven by Haut-Uélé, which drops to 49,9 % after the listing of 131 new contacts, and by Ituri (77,4 %), where Mangala (846 not seen) and Bunia (810) alone account for 67,0 % of the 2 475 contacts not seen.

The alert investigation rate also falls back to 71,2 % at the national level.

EPIDEMIOLOGICAL AND OPERATIONAL CONTEXT

Declared on 15 May 2026, the 17th outbreak of Ebola virus disease (MVE) due to Bundibugyo ebolavirus continues its progression in a complex humanitarian and security context.

Initially confined to the health zone of Mongbwalu (Ituri), it spread to five provinces (Ituri, Nord-Kivu, Sud-Kivu, Haut-Uélé and Tshopo), now affecting 51 health zones.

Population movements, insecurity, artisanal mining activities and cross-border exchanges with Uganda and South Sudan continue to promote transmission.

Multisectoral coordination remains mobilised in order to strengthen surveillance, intensify response interventions and limit the spread of the outbreak.

2.3 Mapping of confirmed cases

Spatial distribution confirms transmission mainly concentrated in Ituri, notably in the health zones of Bunia, Rwampara, Mongbwalu, Nizi, Lita and Nyankunde, with transmission continuing in already affected zones without new geographical extension over the past 24 hours.

Seven health zones in Ituri reported confirmations on 4 August, compared with eleven the day before, but the daily volume increases from 58 to 85 cases.

[FIGURE: FIGURE 2 — SPATIAL DISTRIBUTION OF HEALTH ZONES HAVING REPORTED CONFIRMED CASES]

Transmission is maintained in already affected health zones, which requires simultaneously conducting intensive interventions at the epicentre and preparedness activities in neighbouring provinces.

2.4 Temporal analysis — epidemic curves

The epidemic curve shows sustained transmission during weeks W26 to W31, with fluctuations linked to reporting delays and the progressive consolidation of data.

In Nord-Kivu, W31 is the highest week since the beginning of the outbreak with 101 reported cases, after 82 in W30.

[FIGURE: FIGURE 3 — EPIDEMIC CURVE BY REPORTING WEEK]

[FIGURE: FIGURE 4 EPIDEMIC CURVE BY DATE OF ONSET OF SYMPTOMS (DHIS2 LINE LIST)]

The outbreak remains in a phase of sustained transmission, with a high level of virus circulation.

Recent fluctuations must be interpreted with caution due to reporting delays and the progressive consolidation of data.

2.5 Demographic characteristics

The age pyramid shows a predominance of working-age adults (18–50 years).

Minors under 18 years and people over 50 years are proportionally less affected.

[FIGURE: FIGURE 5 — CONFIRMED CASES BY SEX AND AGE GROUP]

[FIGURE: FIGURE 6 — DEATHS BY SEX AND AGE GROUP (AMONG CONFIRMED CASES)]

The distribution of cases by sex remains balanced.

In Nord-Kivu, 229 confirmed cases are male and 207 female, with the 18–29 and 30–49 age groups bringing together 232 of the 436 cases (53,2 %).

The predominance of cases among active-age adults reflects more frequent community and occupational exposures, whilst the proportion of paediatric cases demonstrates the persistence of intrafamilial transmission.

3.4 Points of entry and control points (PoE/PoC)

3.4.1 — Alerts at crossing points (24 h)

Ituri — 9 alerts notified

Nine alerts were notified at the province’s crossing points: eight living persons and one lifeless body.

Six living alerts, all detected at the PoC of Mudzi Bala (AS Nzere, Bunia) among travellers arriving from Iga-Barrière, were isolated, validated and referred to the Bunia CTE; two were invalidated after secondary screening, at the PoC of Makoko 2 (Mambasa) and at Bunia airport.

The lifeless body, that of a Sudanese national in transit from Ombayi (Ariwara) towards Uganda for burial, was notified and validated but was not swabbed.

The province’s indicators table, for its part, records 8 validated alerts and 7 referred.

Nord-Kivu — 1 alert notified

A living alert was notified at the PoC of Mukulia and invalidated after in-depth investigation.

Reporting completeness stands at 88,9 % (16 PoE/PoC out of 18) and 97,5 % of travellers were screened.

Tshopo — not reported

The PoE/PoC pillar did not present data dated 4 August; the indicators for the province are entered as ND.

Haut-Uélé — 0 alerts notified

No alerts were notified at the activated PoE/PoC of the province.

Eight of the ten activated PoE/PoC reported (80 %) and 10 417 travellers were screened, with the PoC of Magambe accounting for 2 780 on its own (27 %); the PoC of Dusu and Apodo did not transmit their morning data.

The province has only 10 activated PoE/PoC out of the 33 priority ones and no alert has been notified there since the beginning of the outbreak.

7 Main challenges, impacts and required actions

N° Challenges Impact Required actions
1 Reluctance towards post-mortem sampling (EDS/swabs), community resistance and insecurity targeting EDS teams (attack at Muchanga bridge, paralysis of activities in Nyankunde) Confirmation delay, underestimation of cases, disruption of key activities Intensify CREC, involve religious and community leaders, secure teams, increase the number of EDS teams
2 Non-payment of providers (strike in the ZS of Bunia and Rwampara; resumption announced following the intervention of the military governor of Ituri) Interruption of response activities, including dignified and safe burials Regularise the payment of providers; secure dedicated funding
3 Insufficient contact tracing performance: Ituri 77,4 %, Nord-Kivu 74,1 %, Haut-Uélé 49,9 % and Sud-Kivu 100 %, Tshopo not having reported; the ZS of Bambu, Damas, Kambala, Kilo and Lolwa (Ituri) remained with no contacts listed even though they have cumulative cases Chains of transmission not interrupted Train, deploy and motivate community health workers (RECO); reinforced supervision; consolidate the recovery observed in Ituri
4 Insufficiency of ambulances and prepositioning at PoCs Delays in transfer and isolation Preposition ambulances (including at PoE/PoC), deploy CTs per ZS
5 Insufficiency of PCI inputs (PPE, chlorine) and limited functional triage units (182/1 170, or 15 %) High risk of nosocomial infections (140 PPL cases including 43 deaths in Ituri; 14 PPL including 3 deaths in Nord-Kivu and 3 health workers among confirmed cases in Haut-Uélé) Urgent supply, strengthening of PCI training
6 Geographical extension (Haut-Uélé, Tshopo) and high mobility Risk of urban, riverine and cross-border diffusion Activate the 23 priority PoE/PoC in Haut-Uélé; reinforce cross-border coordination
7 Installation of local laboratories (Rwampara, Fataki, Bambu-mine, Boga and Mambasa) Long confirmation delays, undetected transmission Accelerate deployment of decentralised diagnostic capacities
8 Insufficiency of financial resources Limitation of pillar implementation Urgent resource mobilisation, advocacy with partners
9 Insecurity and limited access (CODECO, ADF) Restriction of operations Strengthening of security coordination and humanitarian access
10 Continuity of essential health services (CEHS) compromised Increase in indirect (non-Ebola) mortality Effective implementation of free healthcare, strengthening of CEHS
