1. COORDINATION

1.1. Main Actions

• Monitoring and support of response actions in all affected provinces.

2. RESPONSE ACTIONS BY PILLAR

1.1.1. Epidemiological surveillance

1.1.2. Main Actions

Among the 7 affected provinces, 6 of them recorded a total of 1 978 alerts for the day of 12 September 2026, of which 1 801 (91,1%) were verified. Thus, 445 alerts were validated as suspected cases. The latter were all investigated (100 %), of which 197 were transferred to CT (transit centre)/CTE (Table 3).

Regarding the proportion of contact monitoring, it stands at 91,3% (24 476/26 816), higher both than the previous day (87,5%) and the threshold of 85 %; 88,7% in Ituri (13 230/14 920), 87,6% in Nord-Kivu (10 780/12 310), 83,4% in Bas-Uélé (151/181), 82,0 % in Tshopo (209/255) and 70,7% in Haut-Uélé (106/150).

1.1.3. Challenges

Sub-optimal contact monitoring in 3 provinces out of the 5 having contacts under monitoring with a significant drop in the province of Haut-Uélé, caused by low completeness.

1.2. Update on control points and points of entry activities (PoE/PoC)

1.2.1. Main actions

Six (6) alerts were notified at the various PoE/PoC for the day of 12 September, including five (5) in Ituri and one (1) in Haut-Uélé; all concerned lifeless bodies and no live alert was validated. In Ituri, three bodies were intercepted at the PoC of Kasenyi (ZS (health zone) of Tchomia) and two in the ZS of Mahagi (PoC of Anzida and Barrière DGDA); two of them were swabbed with negative results, the three others having passed without a swab document, representing a proportion of swabbed/secured lifeless bodies of 40 % (2/5). In Haut-Uélé, a lifeless body was intercepted at the PoC of PK10 Route Poko (ZS of Isiro), isolated, swabbed and validated with a negative result (100 %).

1.5. Holistic care

1.5.1. Main actions

• In Ituri, 131 new admissions in the various dedicated facilities against 88 discharges including 13 cured. Thus, 476 patients are hospitalised for 1015 beds, representing an occupancy rate of 46,9% ; 435/463 (94 %) patients received hot meals ; 11/11 (100 %) children are on LNPE/SLM at the CTE, 5/5 (100 %) at the crèche and 28/28 (100 %) in the community.

• In Nord-Kivu, 82 new admissions against 60 recorded discharges including 2 cured; 342 patients are hospitalised, representing an overall occupancy rate in oversaturation (150,0%), with no bed available in the CTE/CT of Butembo and Katwa; 64/64 patients (100 %), 8/8 carers and 52/52 frontline providers received hot meals.

• In Haut-Uélé, 11 new admissions against 9 discharges (5 cured). Thus, 69 patients are hospitalised for 134 beds, representing an occupancy rate of 51,5% ; 42 patients, 55 carers and 26 PPL (person released from treatment (discharged patient)) received hot meals, with the exception of the CTE of Boma Mangbetu, Rungu and Gombari ; start of prophylactic and specific treatment at the CTE of Isiro by the INRB team.

• In Bas-Uélé, 3 new admissions against 2 discharges (1 non-case and 1 absconder); 7 patients remain in isolation including 1 confirmed case managed at the HGR of Ganga, 11 high-risk contacts are on prophylaxis in Buta and in Ganga, for a management data completeness of 38 % (5/13).

• In Tshopo, 9 new admissions were recorded against 3 discharges. Twelve (12) patients are in isolation for 31 beds, representing an occupancy rate of 38,7%.

• In Sud-Kivu, one (1) new admission was recorded, no discharge reported. Seventeen (17) patients are in isolation for 25 beds, representing an occupancy rate of 68,0%.

1.5.2. Challenges

Care still partially outside standards and limited capacities: In Ituri, CTE/CT saturated on the suspect side: Nizi, Mambasa, Lolwa, Kilo, Mangala and Mandima. In Nord-Kivu, oversaturation of facilities (150,0 %) with 49 % of patients managed outside CTE. In Haut-Uélé, the CTE of Pawa lacks hygienic facilities and that of Isiro lacks space, water and permanent electricity. In Tshopo, the absence of safe blood and a limited technical platform weigh on case management. In Bas-Uélé, the absence of CTE/CT and ambulance limits the isolation of cases.

1.6. Vaccination

1.6.1. Main actions

• In Ituri, end of the training of vaccinators and sensitisers as part of preparations for the vaccination of PPL, the start of which is scheduled for 15 September 2026 in Bunia.

1.6.2. Challenges

In Bas-Uélé, the stock-out of ERVEBO in Buta causes a temporary suspension of vaccination of PPL and TPL.

1.7. Continuity of Care

• In Tshopo, 119 cases received the experimental treatment, including 14 confirmed and 105 on post-exposure prophylaxis (68 completed treatments, 11 interrupted and 26 ongoing) ; transfer of 3 high-risk contacts from the CTE to the transit centre of HGR Makiso-Kisangani and briefing of CREC actors on the functioning of the CTE/CT. In Haut-Uélé, briefing of providers on the use of specific and prophylactic treatment of MVE and obtaining, with the INRB team, informed consent from positive patients with a view to the addition of a new molecule.

1.8. Risk communication and community engagement

1.8.1. Main actions

• In Ituri, 14 833 home visits reached 50 031 people and 746 educational talks helped reach 13 640 people; 46 mass sensitisation actions reached 6 553 people, 2 community debates 66 people and 7 advocacy actions 29 people; 172 community alerts were notified (including 20 deaths), 7 of the 18 feedbacks resolved within 48 hours and the 2 community incidents recorded resolved in Nizi and Nia-Nia; in support of pillars, pre-listing of 181 contacts around 13 confirmed cases, facilitation of decontamination of 46 households and one healthcare facility, 10/15 EDS (67 %) and announcement of 14 positive results

• In Nord-Kivu, 41 093 people reached during 17 000 home visits, 2 341 people reached during 68 educational talks, 3 996 during 23 mass communication actions and 88 during 2 briefings; 174 community alerts notified out of 450 expected (38,6 %), 7 feedbacks resolved out of 13 (53,8 %), 1 186 people engaged out of 4 750 (24,9 %) and 3 community incidents resolved (33,3 %).

• In Bas-Uélé, 2 469 people were sensitised in school settings (909 men and 1 560 women); briefing and advocacy with 19 priests of the diocese of Buta, sensitisation of 10 members of the COGE of the health zone of Ganga and support to the briefing of 39 healthcare providers of Buta.

• In Tshopo, 388 visited households reached 2 139 people, 2 educational talks 44 people in Isangi and mass sensitisation actions 1 871 people in the 5 health zones of Kisangani and 6 school institutions; 8 spots broadcast in 4 media outlets; 13 community alerts notified and all resolved, as well as 3 feedbacks and 1 community incident resolved within 48 hours.

• In Haut-Uélé, 43 educational talks reached 587 people and 254 home visits 1 285 people in the ZS of Wamba and Pawa; 4 mass actions reached 411 people at the CTE of Wamba and 1 community dialogue 61 people in Isiro, whilst dialogue conducted around the latest confirmed cases reached 59 people; 15 alerts of suspected cases and deaths notified by community actors, 18 feedbacks resolved within 48 hours, 2 344 people engaged and 1 resistance incident resolved in Isiro; joint preparation for the discharge of 17 conquerors continues in Pawa, Wamba and Gombari.

1.8.2. Challenges

Persistence of community resistance: in Ituri, cases of resistance to EDS are recorded repeatedly in Nizi, to which are added insufficient resources to support the other pillars (mobility, a broken-down printer and the absence of internet connection). In Nord-Kivu, demotivation of CREC actors unpaid for more than three months, non-alignment of certain partners with provincial planning, multiplicity of refusals, resistance and sometimes violent community incidents. In Tshopo, threat targeting a RECO (community health worker) who alerted about a community death and an absconded confirmed case from the CTE still at home. In Bas-Uélé, lack of transport resources and demotivation of

CREC actors, poor forwarding of reports from health zones and impassability of roads.

1.9. Mental health and psychosocial support (SMSPS (mental health and psychosocial support))

1.9.1. Main actions

• In Ituri, 15 people among the new confirmed cases and 103 of the 151 confirmed cases under monitoring (68 %) received SMSPS interventions, as well as 40 of the 116 new suspects (34 %) and 104 of the 137 suspects under monitoring (76 %) ; 56 announcements of results conducted including 15 positive, 67 PPL supported and 39 households accompanied, 25 children monitored, 6/6 children monitored in crèche and 12 of the 16 cured (75 %) followed up in the community.

• In Tshopo, 1 new confirmed case, 2 confirmed cases under monitoring, 3 new suspected cases and 6 suspects under monitoring received SMSPS interventions ; 5 announcements of results conducted including 1 positive ; 13 carers at the CTE, 5 PPL and 25 contacts supported at HGR Makiso-Kisangani, psychological preparation of the EDS team and 2 cured persons having integrated the response teams.

1.9.2. In Haut-Uélé, 6 new confirmed cases and 24 confirmed cases under monitoring, 5 new suspected cases and 8 suspects under monitoring received SMSPS interventions, as well as 70 carers at the CTE and 35 PPL ; 28 laboratory results were announced including 10 positive ; 4 cured persons (3 in Isiro and 1 in Wamba) were discharged without a discharge kit.

1.9.3. Challenges

• In Ituri, low coverage of SMSPS interventions around the latest confirmed cases, only 5 of the 9 reporting health zones being covered, and need to strengthen collaboration with other pillars in Bunia and Komanda. In Tshopo, absence of SMSPS providers in Wanie-Rukula, Bafwasende and Isangi and low participation of partners in pillar activities.

1.10. Logistics

▪ Performance remains consolidated in epidemiological investigations (100 % of suspected cases investigated) as well as an increase in the daily proportion of contact monitoring (88,0%) similar to the previous day (87,5%) which remains above the threshold of 85,0%.

▪ Sustained transmission is noted in the provinces of Ituri, Nord-Kivu and Haut-Uélé whilst the provinces of Tshopo and Bas-Uélé report sporadically.

▪ Over the last 24 hours, 87 new confirmed cases including 28 community deaths and 10 intra-CTE were reported, bringing the national cumulative total to 7 200 confirmed cases and 3 475 deaths (case fatality rate: 48,3%).

▪ Ituri remains the epicentre of the outbreak, concentrating 78,0% of cumulative confirmed cases and 50,6% of new cases reported over the last 24 hours.

▪ Strengthening priority interventions (surveillance, care, PCI, logistics and community engagement) remains essential to quickly interrupt transmission chains: Rapidly increase admission capacities in CT/CTE in Nord-Kivu.
